Working for Queensland Health as an overseas OT, speech pathologist or physio: grades, HHSs, sponsorship and applying
Queensland Health pays allied health clinicians on published Health Practitioner (HP) grades: HP3 runs from A$83,824 to A$120,740 a year and HP4 (senior) from A$132,689 to A$142,315, from 1 September 2026. It’s made up of 16 Hospital and Health Services, and it sponsors overseas clinicians case by case, not as a rule.
I’ve worked in government roles and rural hospitals, and I was the inaugural director of a hospital. This page sets out how Queensland Health grades, pays and recruits allied health clinicians, taken from its own pages and adverts.
The HP grades, and what they pay
Queensland Health publishes its rates. From 1 September 2026:
| Grade | What Queensland Health says the grade involves | Annual salary |
|---|---|---|
| HP3 | Competent clinician; routine practice independently and in a team (includes graduate roles) | A$83,824 (HP3.0) to A$120,740 (HP3.7) |
| HP4 | High level of knowledge and skills with clinical leadership, or managing small teams | A$132,689–142,315 |
| HP5 | Specialised clinical tasks and responsibilities; may carry a high level of formal management | A$149,360–155,647 |
| HP6 | As HP5 | A$165,822–171,483 |
| HP7 | As HP5 | A$188,128–201,251 |
A few notes on reading it:
- Queensland Health’s career structure page groups HP5 to HP8 together and doesn’t describe HP5 and HP6 separately.
- There’s an HP3.8 pay point (A$124,314), but it’s restricted to staff classified a particular way under the 2003 award on 3 January 2008. New starters won’t get it.
- Annual figures are the fortnightly rate × 26.089. The published rates are the award rates where the 2026 State Wage Case put the award above the agreement.
For comparison, the national award minimum (Health Professionals and Support Services Award, from 1 October 2026) runs from A$1,308.80 a week at Level 1 year 1 to A$2,499.10 a week at Level 4. That’s the floor for private employers covered by the award. If you’re working out what grade your current job matches, see how your seniority transfers.

Where the OT vacancies are
A Queensland Parliament answer counted 68.82 full-time-equivalent OT vacancies in April 2026; the breakdown by HHS is in the OT jobs guide. I don’t have equivalent official vacancy figures for speech pathology or physiotherapy.
How applications work
Queensland Health roles are advertised on Queensland Government job sites (Smart Jobs and Queensland Health’s own recruitment site). Three types of listing matter:
- Specific vacancies. One role, one HHS, a closing date.
- Talent pools. An advert that stays open for a set period, rather than for one role. The current Allied Health Talent Pool (OT, physio, speech pathology and others) closes on 15 November 2026 and lists “Australian Working Rights” as a requirement. A Townsville HHS clinical talent pool for speech pathologists (HP3) and senior speech pathologists (HP4) closes on 22 February 2027.
- Graduate programs. A graduate OT (HP3) advert for first-year roles in 2026 closed on 26 September 2025, so look well ahead of the year you want to start.
Expect pre-employment checks. Children’s Health Queensland, for example, lists registration or membership, a Working with Children Check, mandatory vaccinations and a criminal history check, plus at least two references. HHS pages say internationally qualified candidates must be able to register to practise in Queensland.
Does Queensland Health sponsor visas?
Sometimes, case by case. General information only; a registered migration agent should confirm the visa side for you.
- Queensland Health’s visas page says it “may be able to support” a visa if you’re filling a skilled clinical vacancy. That’s for some professions, case by case: the page lists which ones, and I couldn’t confirm which allied health professions are on it, so check the list on the visas page. The classes it “usually” sponsors are the 482, 186 and 494.
- HHS pages (Sunshine Coast, Cairns and Hinterland, CHQ) say: “We sometimes sponsor health professionals, but you can also apply for some visas yourself.” They also say a non-citizen without permanent residency may be appointed to a clinical position “if there aren’t any qualified and experienced people available”. Treat both as something to ask about, not a promise.
- Children’s Health Queensland says you don’t need to be a citizen or permanent resident to apply for a temporary role.
- The 2024 international recruitment campaign that advertised sponsorship closed on 31 October 2024. Some of its pages are still online, but it isn’t current.
In practice, the general talent pool asks for existing work rights, so sponsorship is the exception. If you need it, raise it for the specific role. For how sponsorship works across employers, see visa sponsorship jobs, and check the official detail with Home Affairs.
Graduate programs and work rights: the rules conflict
If you’re a new graduate, two official sources say different things:
- The Queensland Government graduate portal (Smart Jobs) says you must hold Australian or New Zealand citizenship (residing in Australia) or Australian permanent residency to apply. Read literally, a temporary visa holder doesn’t meet that.
- The West Moreton Allied Health Graduate Program in Mental Health (OT, speech pathology and others) also accepts applicants who “have a visa with unrestricted working rights under Australian immigration law”. International applicants must show their visa and work entitlements before starting. Its 2027 intake has closed.
Neither source says whether a working holiday visa counts as “unrestricted working rights”. I won’t guess. If you’re on a temporary visa, check the eligibility wording on the specific program, and ask the program contact before you apply. For other options as a new graduate, see the UK and Irish new-grad OT guide.
Your profession at Queensland Health
- Occupational therapists: registration with the Occupational Therapy Board of Australia (Ahpra) first: A$250 application plus A$138 registration (A$130 in NSW), from 1 October 2026, plus A$688 if you need a portfolio assessment. OTs are employed on the HP grades above, with graduate roles at HP3. See OT jobs in Australia.
- Speech pathologists: you’re not registered with Ahpra. Queensland Health asks for a speech pathology qualification and eligibility for Speech Pathology Australia membership, and overseas-qualified speech pathologists need SPA to assess their qualifications (A$825 through the mutual recognition route, which Canadian S-LPs can’t currently use, or A$2,000 across the four stages of the full assessment). Queensland Health employs speech pathologists at HP3 and senior speech pathologists at HP4. See the speech pathology guides.
- Physiotherapists: assessment by the Australian Physiotherapy Council (Express FLYR A$1,742 for UK, Ireland, Canada, South Africa and Hong Kong, if you’ve held unrestricted registration in the past five years; FLYR A$3,490, including US graduates from Texas, Massachusetts or California; APEP A$8,321 for everyone else; Trans-Tasman for NZ), then registration with the Physiotherapy Board (A$452 application plus A$220 registration, A$200 in NSW, from 1 October 2026). Queensland Health advertises physio roles on the HP grades, for example a senior physiotherapist at HP4 and an advanced physiotherapist at HP5. The Allied Health Talent Pool includes physiotherapy. See the Physiotherapy Council.
Your next step
- OTs: check your OT Board route, and whether you have work rights or need a role that offers sponsorship.
- Speech pathologists: check whether you’re eligible for SPA’s mutual recognition route or need the full assessment, so you can show eligibility for membership when you apply.
- Physios: check which Physiotherapy Council route applies to where you qualified.
The pathway check below sorts that out in a few minutes, so you apply to Queensland Health with the right paperwork and know whether to target talent pools or sponsored vacancies.
